What is an Implant Card?
A patient implant card that meets the following requirements:
- name of the device; and
- model of the device; and
- batch code, lot number or serial number of the device; and
- manufacturer’s name, address and website.
A patient information leaflet that meets the following requirements:
- information identifying the device, or the kind of deviced
- the intended purpose of the device
- information explaining how to use the device safely, and
- other information about the device that the manufacture considers would be useful for patients.
Yes. From 1 December 2021 all implantable and Active Implantable Medical Devices (AIMD) are required to have patient information materials available in the form of both Patient Information Leaflets (PILs) and Patient Implant Cards (PICs), unless specifically excluded from these requirements. PILs and PICs must meet the Essential Principles to be considered compliant.

We provide a convenient way to log and submit treatment information for your patient, allowing easy access to this information for both the clinic and the patient.
Yes, based on the information your practitioners enter when creating a new treatment, the patient will be sent multiple ways to view their implant card. These digital implant cards will be sent via email &/or SMS.
We do not directly supply the patient information leaflet, however we do provide a link to the relevant manufacturers website where the patient can find the leaflets for their treatment. A patient will see this information when viewing their implant card online.

Yes, you can select to create a Patient Consent Form when submitting a treatment, doing so gives you some specific options to customise the consent form. The patient will be required to sign this form on the spot. The patient will then be emailed a PDF version of their signed consent form after the treatment has been submitted.
Yes. You may upload a completed individual consent form when creating a treatment, this form will be sent in the email that the patient will receive once the treatment has been submitted.
Yes. We are an Australian company and all saved treatment data is located on Australian servers.
Yes. We allow one (1) dermal-filler, one (1) anti-wrinkle and one (1) hyalase after-care leaflet. You can replace these files at any time. Submitting a treatment will attach these files to the email sent to the patient.
We can send an sms to the patient with a link to view their implant card online. We cannot attach PDFs to an SMS, this information will be emailed to the patient instead. SMS is only provided in our Plus subscription packages.
We will send an automated email to your patient when you have submitted a treatment. The email will have the implant card attached as an PDF as well as any after-care leaflets you have uploaded to your account.
A patient will be able to login using the email you provided in the treatment submission. Here a patient can view their treatment history, and download any implant cards or after care treatments. Logging into myCosmediCard is completely optional for a patient.

Subscription
Yes, upon subscribing to myCosmediCard you will be given 14 days to test our service, you can cancel before this trial ends or you will be charged for the subscription you choose.
We use Stripe as our payment gateway and currently only accept VISA, and MasterCard payments.
You can subscribe to myCosmediCard with a monthly or yearly payment plan. Yearly payment plans receive a discount.
Absolutely, you can upgrade your plan immediately and you will be charged the difference between your current plan and your new plan.
Yes. However your downgraded plan will not start until the next billing cycle.
Yes. Your subscription will end at the current billing cycle.
No. Payments and subscriptions are final so make sure you are happy with our product within your free 14 day trial.
Your data will not be deleted. So any treatments submitted are still viewable by your patients. Your account will be placed into read-only mode, with few features available. You may optionally request to have your account de-activated so that no one can login to the account.
