Step 1

Start a New Treatment
Once you have finished setting up your account you will be able to submit a new treatment.

1. Simply click the 'New Treatment' in the menu bar.

Step 2

Enter Clinic and Patient Data
You will need to fill out patient data, if a patient has had a treatment uploaded to mycosmedicard with you their data can autofill!

1. Enter information for all required fields (*)
2. You may optionally start the Patient Consent Form creation process too
3. If you have a 'Plus' subscription you will have the option to SMS the finalised treatment to the patient.
4. Once filled, click 'NEXT' and you will be taken to the filler form (or the consent form if you chose to do so)

Step 2A: Optional

If you selected to use our Consent Forms
You can select the consent forms you need for this treatment

1. Select the required Consent Forms (*)
2. Tick that you agree with our terms for using the consent forms
3. Click 'NEXT' to continue

Step 2B: Optional

If you selected to use a Dermal-Filler, Hyalase or Anti-Wrinkle Consent Form
Patient will need to tick they agree with the consent form then sign.

1. Hand device to Patient
2. Patient must tick that they agree with the consent form
3. Patient must sign the form
4. Click 'I AGREE' to continue

Step 2C: Optional

If you selected to use our Media Consent Form
Patient will need to tick they agree with the consent form then sign.

1. Hand device to Patient
2. Patient must tick that they agree with the consent form
3. Patient must sign the form
4. Click 'I AGREE' to continue

Step 3

Add Dermal-Filler, Anti-Wrinkle and/or Hyalase Treatments
Add the treatment data here, so make sure you have this information handy.

1. Select the treatment you need to add tab
2. Enter any information that is required (*)
3. Click 'ADD', you can add as many treatments as you need by repeating 1 - 3.
4. Once Finished click 'SUBMIT'.

Submitting will send an email to the patient with their digital implant card attached as a PDF as well as an SMS if you selected this option.

Thats It! Easy Right?!

Your patient is now all set. They will receive an email with a PDF attachment of their digital implant card based on the information you entered for the treatment. They will also receive any patient care information you have uploaded, this can be found in the 'After Care' section in your account settings. You can upload 1 Dermal-Filler and 1 Anti-Wrinkle After Care, these files can be replaced anytime.

If your account is setup with a Plus subscription, your client can also receive an sms with a link to view their implant card online.

Clinicians can view history of submitted treatments, and easily view all patients in the myCosmediCard portal.

Patients can login to mycosmedicard with their own account access, generated by your treatment submission, where they can view their implant card history.
